The survey

Thank you for taking part in the Organisation Wellness Survey. Your answers are confidential and are not processed or stored individually.

Data are collected and collated by Corporate Heart and your views are combined with others so no one should be able to discover how you personally responded. From this data we prepare an aggregated summary report for your organisation.

Please note that the Organisation Wellness Survey does contain a section in which there are four free-form questions to which you may freely respond. We may in certain circumstances incorporate your answers to these questions verbatim, or summarise or edit your answers, or combine them with other similar responses, in the report which we compile for your organisation.

We will use your answers only for the purpose of the report which we prepare for your organisation and will not disclose them except as set out in these Terms of Survey (or in the limited circumstances where disclosure would be permitted under the Data Protection Act 1998).

We should make clear however that the Organisation Wellness Survey is not an appropriate forum in which to air any personal grievances which you may have about any particular person. Any such grievances should be dealt with in accordance with your organisation's grievance procedure.

The survey has 4 demographic questions, 84 general questions and 4 free-form questions for you to answer and should take no longer than 20-25 minutes to complete. You will be asked to answer all the questions by clicking on the button that most reflects your own experience.

• When you have finished the survey the window will close and the information will be added to the summary report
